Grant funded Rides Increasing Access to Healthcare

The ability to get to medical appointments is vitally important to improving patient health outcomes, reducing emergency department visits, and hospital readmissions.

In collaboration with Hunterdon Healthcare, goHunterdon secured a grant from the Horizon Foundation for New Jersey to provide rides to non-emergency medical appointments for patients of the Hunterdon Wound Healing Center. Patients are adults and older adults without access to transportation.

The project expands upon an existing effort to provide non-emergency medical transportation to Hunterdon Medical Center for transportation dependent patients.
More than 246 rides have been provided via Lyft and local taxi services since the program launched in January 2020. As a result, 147 appointments~ 98% of appointments scheduled have been kept.
As a result of these successes, the program has been expanded to provide rides for older adult patients of primary care offices within the Hunterdon Healthcare System. Patients are referred by Care Coordinators at participating practices.
goHunterdon intends to apply for additional grant funds through the Horizon Foundation for New Jersey to continue to the program for 2021.
